Immunization Requirements by Grade
* Kindergarten:
Shot records must be up to date prior to entering school in August. Parents must bring a copy of the shot record to the school or the Back to School Fair.
* Required/Standard Immunizations: DTaP / Polio, MMR / Varicella, Hepatitis B.
* Additional Recommended/Listed Immunizations: Hepatitis A, Pneumococcal.
* 8th Grade:
Students must receive a Tdap and a Meningococcal vaccination BEFORE attending school in August. HPV is also available.
* 12th Grade (Seniors):
Students must receive a Meningococcal vaccination BEFORE attending school in August. Meningococcal B and HPV are also available at this age.
Annual Sports Physicals
* Requirement: Puxico R-8 School requires an annual sports physical on file before a student can practice or participate in any athletic programs. Obtaining a physical in August satisfies the requirement for all sports throughout the entire school year.
* FREE Physicals Event:
* Location: Back to School Fair hosted by First Baptist Church.
* Date & Time: Saturday, August 8, from 8:00 AM – 12:00 PM.
